About the Role
The Health Records Clerk is responsible for the accurate and timely handling, processing and maintenance of patient health information within both paper-based and electronic health record systems. This role ensures that all records management activities adhere strictly to established departmental procedures and privacy protocols.
The Health Records Clerk will:
- Collect, sort, prepare, scan and accurately file all incoming patient health records and documentation (e.g., test results, physician orders, reports) into the correct physical or electronic patient charts.
- Perform data entry tasks related to patient information, ensuring the accuracy and completeness of certain demographic fields and clinical data points.
- Process patient status changes within the electronic medical record (EMR), including the accurate completion of discharge procedures after final audit.
- Conduct discharge audits of records upon patient discharge to ensure all required information is present, complete and accurate.
- Operate information retrieval systems to accurately locate, retrieve and re-file patient charts and data as requested by authorized clinical staff for immediate patient care needs.
- Assist senior staff in organizing, arranging, indexing and classifying records to meet immediate business needs and ensure efficient retrieval.
- Catalogue and track releases of information.
- Prepare and securely package information release documents for mailing, couriering or electronic distribution, maintaining strict confidentiality.
- Serve as a primary point of contact for internal staff and physicians regarding basic health records correspondence and filing inquiries.
- Safeguard patient records and ensure strict adherence to all privacy guidelines and regulations when handling sensitive health information.
- Perform additional duties as required.
- Experience in applying records management principles and practices.
- Proficiency in EMR systems and standard office equipment.
- Solid understanding of provincial privacy legislation related to health records (Health Information Act).
- High level of integrity, confidentiality and accountability.
- Strong organizational and time management skills, with the ability to prioritize work in a fast-paced clinical environment.
- Excellent problem solving and interpersonal communication skills.
- Strong attention to detail and a high degree of accuracy.
- Ability to work independently with minimal supervision and collaboratively within a team setting.
- Proficiency in computer use including Microsoft Office Suite and Adobe.
- A high school diploma and a minimum of one year of related experience in a records management focused environment is required.
- A post-secondary diploma or certification in health information management or a directly related field would be considered an asset.
- Directly related work experience or education is considered based on one year of experience for one year of education or one year of education for one year of experience.
- Certification in Health Information Management would be an asset.
Physical Demands
- Manual dexterity to use mouse, keyboard and computer.
- Sitting and standing for extended periods of time.
- Must be able and willing to lift and carry up to 10 pounds.
- Travel between CASA sites.
